Le processus consiste &agrave; pr&eacute;ciser l&rsquo;orientation &agrave; prendre, ainsi que les priorit&eacute;s et les m&eacute;canismes d&rsquo;&eacute;valuation des r&eacute;sultats. Il existe des <a href=\"https:\/\/www.nccmt.ca\/fr\/referentiels-de-connaissances\/interrogez-le-registre\" target=\"_blank\" rel=\"noopener\">m&eacute;thodes et des outils<\/a> pour aider les organismes motiv&eacute;s &agrave; agir sur les d&eacute;terminants sociaux de la sant&eacute; et l&rsquo;&eacute;quit&eacute; en sant&eacute; &agrave; l&rsquo;&eacute;chelle des collectivit&eacute;s et des populations.<\/p>\n<p>Il est en outre possible d&rsquo;am&eacute;liorer les processus en place pour favoriser l&rsquo;&eacute;quit&eacute; en sant&eacute;, notamment en incorporant d&rsquo;autres outils ou &laquo;&thinsp;perspectives&thinsp;&raquo; qui guident les praticiens &agrave; d&eacute;terminer les &eacute;l&eacute;ments cl&eacute;s n&eacute;cessaires pour att&eacute;nuer les iniquit&eacute;s de sant&eacute;.<\/p>\n<h3>\nOutil de planification<\/h3>\n<p>Les conf&eacute;renci&egrave;res de ce webinaire offert en anglais font &eacute;tat de l&rsquo;outil de planification <a href=\"http:\/\/nccdh-dev.media-doc.ca\/resources\/entry\/community-planning-tool-applying-a-health-equity-lens-to-program-planning\" target=\"_blank\" rel=\"noopener\"><em>Community Planning tool&nbsp;: Applying a health equity lens to program planning<\/em><\/a>&nbsp;(en anglais) con&ccedil;u par la <a href=\"https:\/\/www.fraserhealth.ca\/\" target=\"_blank\" rel=\"noopener\">Fraser Health Authority<\/a>, en Colombie-Britannique. L&rsquo;outil sert&nbsp;d&rsquo;exemple pour montrer comment appliquer la perspective de l&rsquo;&eacute;quit&eacute; en sant&eacute; de mani&egrave;re &agrave; enrichir les processus employ&eacute;s &agrave; l&rsquo;heure actuelle pour planifier les programmes.<\/p>\n<p>Les conf&eacute;renci&egrave;res d&eacute;crivent les sept &eacute;tapes du processus de planification, c&rsquo;est-&agrave;-dire&nbsp;:<\/p>\n<ul>\n<li>&eacute;valuer les iniquit&eacute;s;<\/li>\n<li>&eacute;tablir des partenariats;<\/li>\n<li>mobiliser la collectivit&eacute;;<\/li>\n<li>choisir les approches;<\/li>\n<li>mettre en &oelig;uvre et faire le suivi;<\/li>\n<li>&eacute;valuer les progr&egrave;s;<\/li>\n<li>poursuivre sur la lanc&eacute;e.<\/li>\n<\/ul>\n<p>Les conf&eacute;renci&egrave;res font &eacute;tat de certains cas o&ugrave; l&rsquo;outil a &eacute;t&eacute; utilis&eacute;. Elles offrent des conseils sur la mani&egrave;re de r&eacute;aliser chacune des &eacute;tapes. L&rsquo;outil se r&eacute;v&eacute;le&nbsp;utile pour les praticiens de premi&egrave;re ligne et les cadres de direction de tous les champs d&rsquo;activit&eacute; du domaine de la sant&eacute; publique, particuli&egrave;rement les gestionnaires et les directeurs de programmes, ainsi que pour les organismes communautaires.<\/p>\n<h3>\nLe webinaire servira &agrave; expliquer<\/h3>\n<ul>\n<li>les sept &eacute;tapes &agrave; suivre pour planifier des programmes en tenant compte de la perspective de l&rsquo;&eacute;quit&eacute; en sant&eacute;;<\/li>\n<li>la d&eacute;marche &agrave; suivre pour appliquer cet outil aux processus en place pour planifier et ex&eacute;cuter les programmes;<\/li>\n<li>les applications pratiques d&rsquo;un processus &agrave; &eacute;tapes multiples dans une d&eacute;marche pour favoriser l&rsquo;&eacute;quit&eacute;
