Ce faisant, les auteures souhaitent inciter &agrave; l&rsquo;&eacute;laboration d&rsquo;une telle strat&eacute;gie pour l&rsquo;ensemble du Canada. Elles estiment que, m&ecirc;me si le Canada a contribu&eacute; &agrave; la conscientisation au concept des d&eacute;terminants sociaux de la sant&eacute; (DSS), ses politiques ne sont malheureusement pas arriv&eacute;es &agrave; att&eacute;nuer la r&eacute;partition in&eacute;gale de la sant&eacute; et de ses d&eacute;terminants sociaux. Elles font &eacute;galement valoir la n&eacute;cessit&eacute; de mettre en place une strat&eacute;gie nationale, dans une optique &agrave; la fois symbolique et strat&eacute;gique, afin d&rsquo;inciter tous les paliers de gouvernement &agrave; accorder la priorit&eacute; aux disparit&eacute;s en sant&eacute;.<\/p>\n<p>\n\t\tLes strat&eacute;gies nationales diff&egrave;rent consid&eacute;rablement les unes des autres : certaines visent uniquement les in&eacute;galit&eacute;s en sant&eacute;, tandis que d&rsquo;autres int&egrave;grent l&rsquo;&eacute;quit&eacute; en sant&eacute; &agrave; d&rsquo;autres objectifs; certaines rel&egrave;vent du gouvernement central, tandis que d&rsquo;autres incombent au minist&egrave;re de la sant&eacute;.<\/p>\n<p>\n\t\tEn Su&egrave;de, le projet de loi sur les objectifs en mati&egrave;re de sant&eacute; publique (2003) &eacute;nonce que tous les minist&egrave;res ont une responsabilit&eacute; &agrave; l&rsquo;&eacute;gard des 11 objectifs ax&eacute;s sur la cr&eacute;ation de conditions sociales &eacute;quitables pour l&rsquo;ensemble de la population. Le minist&egrave;re de la sant&eacute; de la Norv&egrave;ge coordonne l&rsquo;&eacute;laboration de la politique sur l&rsquo;&eacute;quit&eacute; en sant&eacute; avec les politiciens et les fonctionnaires de six autres minist&egrave;res. En Angleterre, les conseils de la sant&eacute; et du mieux-&ecirc;tre coordonnent, par l&rsquo;entremise d&rsquo;autorit&eacute;s locales, l&rsquo;int&eacute;gration du travail en termes de sant&eacute;, de services sociaux et d&rsquo;att&eacute;nuation des in&eacute;galit&eacute;s en sant&eacute;. En &Eacute;cosse, on a r&eacute;cemment donn&eacute; aux autorit&eacute;s locales plus d&rsquo;autonomie pour ce qui est d&rsquo;&eacute;tablir et de mettre en &oelig;uvre les strat&eacute;gies de mani&egrave;re &agrave; ce qu&rsquo;elles tiennent compte des valeurs et des besoins de la collectivit&eacute;. &nbsp;<\/p>\n<p>\n\t\tLes auteures d&eacute;crivent en outre deux initiatives r&eacute;alis&eacute;es &agrave; l&rsquo;&eacute;chelle europ&eacute;enne : <a href=\"http:\/\/www.epha.org\/IMG\/pdf\/European_Charter_Health_Equity_July_v.pdf\" onclick=\"window.open(this.href, '', 'resizable=no,status=no,location=no,toolbar=no,menubar=no,fullscreen=no,scrollbars=no,dependent=no'); return false;\">la European Charter for Health Equity<\/a>&nbsp; (en anglais; charte europ&eacute;enne pour l&rsquo;&eacute;quit&eacute; en sant&eacute;; 2011) et<a href=\"http:\/\/www.health-inequalities.eu\/HEALTHEQUITY\/EN\/home\/\" onclick=\"window.open(this.href, '', 'resizable=no,status=no,location=no,toolbar=no,menubar=no,fullscreen=no,scrollbars=no,dependent=no'); return false;\"> le European Portal for Action on Health Inequalities<\/a>&nbsp; (en anglais; portail europ&eacute;en pour une action sur les in&eacute;galit&eacute;s en sant&eacute;; 2011). Le portail offre un espace propice &agrave; l&rsquo;&eacute;change d&rsquo;information, de politiques et de pratiques exemplaires en mati&egrave;re de d&eacute;terminants sociaux de la sant&eacute; et de la sant&eacute; dans toutes les politiques.<\/p>\n<p>\n\t\t<strong>Selon Haber et Wong, une strat&eacute;gie nationale en mati&egrave;re d&rsquo;&eacute;quit&eacute; en sant&eacute; doit pr&eacute;senter les caract&eacute;ristiques suivantes pour &ecirc;tre efficace :<\/strong><\/p>\n<ul>\n<li>\n\t\t\t&ecirc;tre ax&eacute;e sur les d&eacute;terminants sociaux de la sant&eacute;;<\/li>\n<li>\n\t\t\t&eacute;tablir judicieusement les cibles et les &eacute;tapes d&rsquo;&eacute;valuation;<\/li>\n<li>\n\t\t\td&eacute;terminer des m&eacute;canismes en vue d&rsquo;orienter et de coordonner les politiques au sein et &agrave; l&rsquo;ext&eacute;rieur du gouvernement;<\/li>\n<li>\n\t\t\tmontrer des rapports &eacute;troits entre les politiques et les programmes aux &eacute;chelons national et local;<\/li>\n<li>\n\t\t\tavoir une coordination entre les diff&eacute;rents acteurs gouvernementaux charg&eacute;s des politiques autres que celles touchant la sant&eacute;.<\/li>\n<\/ul>\n<p>\n\t\t<strong>Servez-vous de cet outil pour :<\/strong><\/p>\n<ul>\n<li>\n\t\t\targumenter en faveur d&rsquo;une strat&eacute;gie nationale en mati&egrave;re d&rsquo;&eacute;quit&eacute; en sant&eacute; au Canada;<\/li>\n<li>\n\t\t\tcomprendre comment il est possible de coordonner les initiatives nationales, provinciales et locales gr&acirc;ce &agrave; une politique nationale;<\/li>\n<li>\n\t\t\tconsid&eacute;rer les options et les facteurs de r&eacute;ussite li&eacute;s aux politiques con&ccedil;ues pour r&eacute;duire les iniquit&eacute;s sociales entra&icirc;nant les disparit&eacute;s en sant&eacute;.<\/li>\n<\/ul>\n<p>\n\t\tHaber, R. et E.
