{"id":6981,"date":"2016-11-23T11:36:41","date_gmt":"2016-11-23T11:36:41","guid":{"rendered":"http:\/\/nccdh.local\/resource\/improving-population-health-by-working-with-communities-action-guide-3-0\/"},"modified":"2025-11-06T17:20:07","modified_gmt":"2025-11-06T17:20:07","slug":"improving-population-health-by-working-with-communities-action-guide-3-0","status":"publish","type":"resource","link":"https:\/\/nccdh-dev.media-doc.ca\/fr\/resources\/entry\/improving-population-health-by-working-with-communities-action-guide-3-0\/","title":{"rendered":"Improving population health by working with communities: Action guide 3.0"},"content":{"rendered":"<p>\n\t\tLe guide d&rsquo;intervention du National Quality Forum comporte un cadre d&rsquo;action visant &agrave; aider les groupes d&rsquo;intervenants plurisectoriels &agrave; se mobiliser pour am&eacute;liorer la sant&eacute; des populations en s&rsquo;appuyant sur 10 facteurs de r&eacute;ussite interd&eacute;pendants et en ayant recours &agrave; des ressources connexes au besoin.<\/p>\n<p>\n\t\tSemblable &agrave; un mode d&rsquo;emploi, le guide d&rsquo;intervention est structur&eacute; en fonction des facteurs de r&eacute;ussite. Il comporte des d&eacute;finitions, des recommandations, des exemples pratiques et diverses suggestions de ressources destin&eacute;s &agrave; aider les collectivit&eacute;s &agrave; atteindre leurs objectifs communs et &agrave; am&eacute;liorer de mani&egrave;re durable la sant&eacute; de leurs populations. Les auteurs ont express&eacute;ment gard&eacute; au minimum le nombre de pages et r&eacute;dig&eacute; le contenu dans un style simple et accessible pour que des intervenants de tous horizons (que ce soit sur les plans local, &eacute;tatique, r&eacute;gional ou national) puissent passer &agrave; l&rsquo;action.<\/p>\n<p>\n\t\tPar l&rsquo;entremise de sa National Quality Strategy (strat&eacute;gie nationale en mati&egrave;re de qualit&eacute;), le National Quality Forum met l&rsquo;accent sur l&rsquo;&eacute;nergie et les interventions collectives pour am&eacute;liorer les indicateurs sociaux, &eacute;conomiques et environnementaux qui influent sur la sant&eacute;. Parall&egrave;lement, l&rsquo;organisme reconna&icirc;t que, jusqu&rsquo;&agrave; maintenant, l&rsquo;am&eacute;lioration et l&rsquo;&eacute;valuation de la qualit&eacute; ont essentiellement &eacute;t&eacute; ax&eacute;es sur le syst&egrave;me de soins cliniques. L&rsquo;objectif du projet consiste &agrave; mieux comprendre comment les collectivit&eacute;s peuvent travailler de concert avec les acteurs des syst&egrave;mes de sant&eacute; publique et de soins cliniques pour am&eacute;liorer la sant&eacute; des populations. Les d&eacute;finitions communes et le cadre conceptuel g&eacute;n&eacute;ral visent &agrave; assurer une meilleure coordination et &agrave; favoriser les partenariats dans la collectivit&eacute;.<\/p>\n<p>\n\t\tC&rsquo;est gr&acirc;ce &agrave; une entente avec le d&eacute;partement de la Sant&eacute; et des Services sociaux des &Eacute;tats-Unis que le National Quality Forum a pu financer le projet de trois ans et explorer certaines questions, par exemple :<\/p>\n<ul>\n<li>\n\t\t\t<span style=\"font-size: 12px;\">Comment des individus et des groupes d&rsquo;intervenants de tous horizons peuvent-ils se mobiliser pour am&eacute;liorer la sant&eacute; de la collectivit&eacute;?<\/span><\/li>\n<li>\n\t\t\tQuelles personnes et quels organismes devraient se trouver autour de la table?<\/li>\n<li>\n\t\t\tQuels processus et quelles m&eacute;thodes les collectivit&eacute;s devraient-elles employer pour &eacute;valuer la sant&eacute; de leurs populations?<\/li>\n<li>\n\t\t\tQuelles donn&eacute;es existe-t-il pour &eacute;valuer, analyser et combler les besoins de la collectivit&eacute; en mati&egrave;re de sant&eacute; et mesurer les progr&egrave;s?<\/li>\n<li>\n\t\t\t&Agrave; quelles mesures incitatives peut-on avoir recours pour harmoniser et coordonner les efforts pour am&eacute;liorer la sant&eacute; de la collectivit&eacute;?<\/li>\n<li>\n\t\t\tComment les collectivit&eacute;s peuvent-elles s&rsquo;y prendre pour offrir plus de soins de sant&eacute; &agrave; co&ucirc;t abordable tout en favorisant la coordination, l&rsquo;efficience et la r&eacute;duction des d&eacute;penses?<\/li>\n<\/ul>\n<p>\n\t\t<strong>Servez-vous de cet outil pour :<\/strong><\/p>\n<ul>\n<li>\n\t\t\tentreprendre une auto-&eacute;valuation collaborative des d&eacute;marches en mati&egrave;re de sant&eacute; des populations;<\/li>\n<li>\n\t\t\ttrouver les exemples et les ressources utiles &agrave; la planification et &agrave; l&rsquo;&eacute;tablissement des priorit&eacute;s organisationnelles;<\/li>\n<li>\n\t\t\tr&eacute;unir les acteurs de la collectivit&eacute; et du secteur de la sant&eacute; afin d&rsquo;explorer les possibilit&eacute;s de collaboration.<\/li>\n<\/ul>\n<p>\n\t\t<strong>R&eacute;f&eacute;rence bibliographique :&nbsp;<\/strong><\/p>\n<p>\n\t\tNational Quality Forum.
