Le document <a href=\"http:\/\/www.simcoemuskokahealth.org\/docs\/default-source\/jfy-communities\/sdohfoundationaldocument_-_final_2\" target=\"_blank\" rel=\"noopener\"><em>Simcoe Muskoka District Health Unit&rsquo;s (SMDHU) approach to addressing the determinants of health: A health equity framework<\/em><\/a> (L&rsquo;approche en mati&egrave;re de d&eacute;terminants de la sant&eacute; du Bureau de sant&eacute; du comt&eacute; de Simcoe Muskoka : Un cadre th&eacute;orique pour l&rsquo;&eacute;quit&eacute; en sant&eacute;) a servi de fondation pour le <a href=\"http:\/\/www.simcoemuskokahealth.org\/HealthUnit\/About\/Profile\/StrategicPlan\" target=\"_blank\" rel=\"noopener\">plan strat&eacute;gique 2012-2016 du Bureau<\/a>, lequel visait notamment &agrave; r&eacute;duire les iniquit&eacute;s en sant&eacute; entre les divers groupes de population. Le plan strat&eacute;gique 2019-2022 poursuit dans la m&ecirc;me lign&eacute;e, mettant &agrave; l&rsquo;avant-plan des valeurs comme l&rsquo;&eacute;quit&eacute; et soulignant l&rsquo;importance vitale de l&rsquo;am&eacute;lioration des relations avec les populations prioritaires pour s&rsquo;attaquer aux d&eacute;terminants sociaux de la sant&eacute;.<\/p>\n<h3>\nStructure du cadre th&eacute;orique<\/h3>\n<p>Ce cadre th&eacute;orique a &eacute;t&eacute; &eacute;labor&eacute; afin de soutenir le personnel de sant&eacute; publique du Bureau et d&rsquo;obtenir notamment les r&eacute;sultats suivants :<\/p>\n<ul>\n<li>Accorder une importance appropri&eacute;e aux populations prioritaires susceptibles aux iniquit&eacute;s en sant&eacute; dans la r&eacute;ponse globale des organismes.<\/li>\n<li>Mettre en &oelig;uvre un plan d&rsquo;action pour att&eacute;nuer les conditions de risque afin de r&eacute;pondre aux besoins de sant&eacute; publique de ces populations.<\/li>\n<\/ul>\n<p>Le cadre comprend aussi un glossaire de l&rsquo;&eacute;quit&eacute; en sant&eacute;, des renseignements et des r&eacute;f&eacute;rences pour mieux comprendre les divers mod&egrave;les de d&eacute;terminants sociaux de la sant&eacute;. On y trouve aussi les fondements th&eacute;oriques du cadre.<\/p>\n<p>L&rsquo;une de ses sections porte sur la reconnaissance des populations prioritaires : elle contient des termes, des sources de donn&eacute;es et des m&eacute;thodes indispensables pour d&eacute;terminer qu&rsquo;une population prioritaire est soumise &agrave; de plus grands risques de probl&egrave;mes de sant&eacute; d&eacute;coulant de facteurs socio&eacute;conomiques. Le document met aussi en contexte les influences politiques et gouvernementales sur les priorit&eacute;s des syst&egrave;mes de sant&eacute; publics. De plus, il indique comment d&eacute;terminer les besoins, l&rsquo;incidence des mesures, la capacit&eacute; d&rsquo;agir et les occasions de collaboration.<\/p>\n<h3>\nPertinence pour les ressources du CCNDS<\/h3>\n<p>Diff&eacute;rents exemples de la pertinence des actions pos&eacute;es par le Bureau pour <a href=\"http:\/\/nccdh-dev.media-doc.ca\/fr\/resources\/entry\/lets-talk-public-health-roles\" target=\"_self\" rel=\"noopener\">le r&ocirc;le de la sant&eacute; publique dans l&rsquo;am&eacute;lioration de l&rsquo;&eacute;quit&eacute; en sant&eacute;<\/a> et les <a href=\"http:\/\/127.0.0.1:8001\/fr\/our-work\/\" target=\"_self\" rel=\"noopener\">strat&eacute;gies d&rsquo;am&eacute;lioration des capacit&eacute;s<\/a> du CCNDS illustrent bien en quoi ces principes s&rsquo;appliquent aux programmes et aux priorit&eacute;s de sant&eacute; publique. Le cadre comprend aussi un ensemble de recommandations pour passer &agrave; l&rsquo;action en sant&eacute; publique r&eacute;dig&eacute;es par le personnel du Bureau et plusieurs annexes pr&eacute;sentant les principaux documents organisationnels, des donn&eacute;es sur l&rsquo;&eacute;tat de sant&eacute; de la population de la r&eacute;gion et des exemples d&rsquo;outils externes pour la planification et l&rsquo;&eacute;tablissement de priorit&eacute;s.<\/p>\n<h3>\nServez-vous de cet outil pour<\/h3>\n<ul>\n<li>faciliter les &eacute;changes l&rsquo;int&eacute;grer de l&rsquo;&eacute;quit&eacute; en sant&eacute; aux documents organisationnels en sant&eacute; publique pour en faire une priorit&eacute;;<\/li>\n<li>cr&eacute;er un cadre th&eacute;orique pour redresser les iniquit&eacute;s en sant&eacute; par l&rsquo;&eacute;laboration de programmes et de priorit&eacute;s;<\/li>\n<li>&eacute;valuer comment les mesures de sant&eacute; publique s&rsquo;harmonisent avec le r&ocirc;le que doit jouer le CCNDS en la mati&egrave;re et avec les strat&eacute;gies de renforcement de la capacit&eacute; pour rep&eacute;rer les points &agrave; am&eacute;liorer sur le plan de l&rsquo;&eacute;quit&eacute; en sant&eacute;.<\/li>\n<\/ul>\n<h3>\nR&eacute;ference<\/h3>\n<p>Simcoe Muskoka District Health Unit.
