Lanc&eacute; en 2018, ce projet a pour but d&rsquo;aider les organisations &agrave; am&eacute;liorer leur capacit&eacute; &agrave; agir sur l&rsquo;&eacute;quit&eacute; en sant&eacute; en leur offrant des cadres th&eacute;oriques, des strat&eacute;gies et des conditions organisationnelles pertinents.<\/p>\n<h3>\nPr&eacute;sentation du projet<\/h3>\n<p>Le document r&eacute;sume les objectifs du projet, y compris ceux &agrave; long terme, et les r&eacute;sultats escompt&eacute;s. Les objectifs &agrave; long terme comprennent notamment de fournir des mod&egrave;les pour l&rsquo;action en mati&egrave;re d&rsquo;&eacute;quit&eacute; en sant&eacute; qui pourront &ecirc;tre utilis&eacute;s par tous les organismes de sant&eacute; publique, et non seulement par les deux sites du projet pilote.<\/p>\n<p>La description comprend aussi une d&eacute;finition de la capacit&eacute; organisationnelle comme l&rsquo;entend le CCNDS ainsi qu&rsquo;une explication des volets du programme :<\/p>\n<ul>\n<li>L&rsquo;approche par cercle d&rsquo;apprentissage<\/li>\n<li>Les sites d&rsquo;essai<\/li>\n<li>Les analyses documentaires<\/li>\n<li>Le groupe de r&eacute;flexion<\/li>\n<\/ul>\n<p>Le Sommaire est le premier d&rsquo;une s&eacute;rie de documents r&eacute;dig&eacute;s pour &eacute;tayer le projet; il s&rsquo;agit de la deuxi&egrave;me it&eacute;ration de la s&eacute;rie &laquo; <a href=\"http:\/\/nccdh-dev.media-doc.ca\/fr\/resources\/entry\/population-health-status-reporting\" target=\"_self\" rel=\"noopener\">Apprenons ensemble<\/a> &raquo; du CCNDS.
