Une planification solide peut aider &agrave; voir si un programme en cours risque d&rsquo;aggraver accidentellement les iniquit&eacute;s de sant&eacute; ou peut contribuer &agrave; les r&eacute;duire.<\/p>\n<p>Dans cette optique, la <a href=\"https:\/\/www.fraserhealth.ca\/\" target=\"_blank\" rel=\"noopener\">Fraser Health Authority<\/a> a con&ccedil;u cet outil de planification pour aider les programmes qui cherchent &agrave; aborder les aspects physiques, sociaux, mentaux et &eacute;motionnels de la sant&eacute; dans leur communaut&eacute;.<\/p>\n<h3>Outil en sept &eacute;tapes<\/h3>\n<p>L&rsquo;outil comporte sept &eacute;tapes distinctes pour la planification et la mise en &oelig;uvre de programmes tenant compte des iniquit&eacute;s. Il permet aussi d&rsquo;adapter des interventions pour les communaut&eacute;s qui en ont particuli&egrave;rement besoin.<\/p>\n<p>Voici les sept &eacute;tapes&nbsp;:<\/p>\n<ol>\n<li>&Eacute;valuer les iniquit&eacute;s et d&eacute;finir la port&eacute;e<\/li>\n<li>Rep&eacute;rer les partenariats potentiels et les &eacute;tablir<\/li>\n<li>&Eacute;valuer les capacit&eacute;s de la communaut&eacute; et la mobiliser<\/li>\n<li>Choisir l&rsquo;approche de changement et planifier les mesures &agrave; prendre<\/li>\n<li>Mettre en &oelig;uvre et surveiller<\/li>\n<li>&Eacute;valuer les progr&egrave;s<\/li>\n<li>Poursuivre la lanc&eacute;e<\/li>\n<\/ol>\n<p>L&rsquo;outil peut &ecirc;tre utilis&eacute; par des personnes et des &eacute;quipes. Il comprend&nbsp;:<\/p>\n<ul>\n<li>des feuilles de travail pour guider les praticiens &agrave; chaque &eacute;tape;<\/li>\n<li>un glossaire des termes importants;<\/li>\n<li>des annexes contenant des conseils sur la fa&ccedil;on d&rsquo;accomplir les diff&eacute;rentes t&acirc;ches de chaque &eacute;tape du processus.<\/li>\n<\/ul>\n<p>L&rsquo;outil sera utile pour la planification de programmes &agrave; court et &agrave; long terme visant &agrave; r&eacute;pondre aux besoins de la communaut&eacute; et &agrave; diminuer les iniquit&eacute;s de sant&eacute;.
